Parent/Guardian Survey Consent

Better Futures Project

Thank you for considering your student's participation in one of our Better Futures Projects in the state of Kansas. We have been providing Kansas students with healthy relationship education for three years and have reached over 1,000 students in Family and Consumer Science classrooms.

The Better Futures Projects seek to provide quality evidence-based and medically accurate relationship education to students, helping them have the best start to their adult lives as possible. The projects bring the Love Notes curriculum to your student(s) in a safe and supportive space to allow youth to better understand their values, what makes a healthy relationship, and how to identify an unhealthy relatoinship. Love Notes has been proven to be effective in delaying pre-maritaly sexual activity and preventing teen pregnancy. You can find more about Love Notes on The Dibble Institute website.

To continue bringing Better Futures Projects to Kansas schools and to continually improve the progams, we ask students to voluntarily participate in surveys before and after they have completed Love Notes.
